Chronic Disease Management (CDM)

Overview of Service

In British Columbia, chronic disease management involves health care providers, often working in a team with their office staff and Health Authority resources, to provide health care services to individuals with long-term or chronic conditions. The Ministry has developed a secure, Web-based clinical quality improvement application, supporting doctors and health care teams to use best practices in caring for their patients with chronic diseases.

Purpose of Service

Health care providers in B.C. can use the Ministry's secure Web site to pick up personal chronic disease management patient registers, and use the Chronic Disease Management (CDM) Toolkit, to help with management of their patients with chronic conditions.

Registration

If you are a licensed physician registered with the Medical Services Plan of BC, you are eligible to request access at the Secure Web Site for Practitioners.
