The MacColl Institute for HealthCare Innovation, at Group Health Cooperative in Seattle, supports an extensive Web site on improving chronic illness management. Improving Chronic Illness Care (ICIC) is a national program office of The Robert Wood Johnson Foundation based at The MacColl Institute.

The Chronic Disease Management Model was developed to provide a framework of best practices in managing chronic diseases. The model shows the areas of conventional practice that need to change, and identifies necessary changes to improve the system to support better health outcomes.
