Help with Access Request Form
The Regulated Healthcare Provider Registration Agreement is intended for individual physicians requesting access to the Chronic Disease Management (CDM) Secure Website. If you are a member of a physician group practice, each physician member of the group must complete and submit a separate registration agreement.
The registration agreement is accessible through the website link: http://healthnet.hnet.bc.ca/has/regagree/4614fil.pdf. Please read the terms and conditions in the top half of the form and ensure you are in agreement before completing the Applicant Authorization section and submitting the agreement. Once the form is processed and approved, you will receive an email from Data Access Services (DAS) including a digital certificate for installation on your PC, and a UserID and password for accessing the required services.
The Ministry uses the information submitted on the form to verify the Physician's identity with the BC College of Physicians and Surgeons, and with the Medical Services Plan (MSP), to ensure that the Physician is eligible (licensed) to provide services in BC. If the Ministry cannot process your request, e.g. due to a mismatch between the information submitted on the form, and your records at MSP or the College, you will receive an email indicating the nature of the problem, and a suggestion for how to correct it.
After Completing the Fill-in Fields, Check for Accuracy, and Print the Form
Please ensure that all your fill-in information is correct. Prior to clicking on the Submit button, please click on the Print button to provide you with a printed copy of the completed Registration Agreement, for your records. If you have clicked on Submit before Print, you can use your browser's Back button to move back to the completed registration agreement screen and Print. This printed copy provides you with the Terms & Conditions, and a record of the information you submitted in your registration agreement, and will be helpful should you need to refer to this information at a later date.
After Printing the Form, Click the Submit button Once you have completed (and printed) the form, click on the Submit button. The form will be automatically emailed to the Ministry's Data Access Services (DAS) group for review and approval. It normally only takes a few business days to process your form.
Contacts and Access Requests for your staff: Although a Physician may act as his/her own Contact (by leaving these two fields blank), the Physician may use these fields to designate an individual (e.g., an Office Manager, Nurse, Medical Office Assistant) to have access to the CDM Secure Website, for picking up the Physician's secure data.
A request for additional access for other individuals (any persons over and above a first Contact Name provided in the registration agreement) can be made by contacting Data Access Services (DAS) (hlth.hnetconnection@gov.bc.ca). Details of the information required to request additional user IDs are provided through the 'Confirmation Notice' email you will receive when your access has been approved.
You will require Adobe Acrobat Reader Version 5.0, or higher Successful completion and submission of the web-based Registration Agreement fill-in form requires Adobe Acrobat Reader Version 5.0, or higher, to be installed on your computer. Please ensure you have the required version before proceeding with completion of the form. If you are uncertain which version of Adobe Acrobat Reader your computer is using, open the program and click on Help | About Adobe Reader. The Adobe logo will display the version number. If you need to install, or upgrade to, Adobe Acrobat Reader Version 5.0, you can do so at no charge from the Adobe site at http://www.adobe.com/products/acrobat/readstep.html
