Patient care complaints should be sent to the Board in writing. They can be sent by mail, fax or by using the contact us button.

In order to aid with the investigation of your complaint, please provide as much of the following information as possible.

• details of complaint
• date, time and location of the incident
• patient's name, address and phone number
• your name, address and phone number
• attendants' names (if known)
• hospital transported to
• attending physician's name
