Replace Your CareCard

Lost, Damaged or Stolen Cards

If your card has been lost, damaged or stolen, please complete the HLTH 2811 CareCard Replacement Form (124k fill-and-print PDF). You can also call our 1-800 number (see Contact Us) and select the CareCard Replacement option.

You will need to include your current residential and mailing addresses. Please return any damaged CareCard to MSP."

You will be notified if a fee is required for the replacement card. See Fees for more information.

Name Changes

If you pay your premiums directly and need to change your name, complete a CareCard Replacement Form (pdf). You will need to include:

  • Your current residential and mailing addresses, and
  • A photocopy of the legal name change document (change of name certificate, marriage certificate, birth certificate, citizenship card).

If a replacement card is needed because of a name change and you are covered under a plan administered by your employer, union or pension plan, you need to complete a Group Change Form and submit it to your group plan administrator, along with a copy of the legal document showing your name change.

Fees

If a replacement fee is required, Health Insurance BC will notify you separately of any charge. Please do not send any payments in advance of receiving this notification.
