BC Palliative Care Benefits Program
The BC Palliative Care Benefits Program supports BC residents of any age who have reached the end stage of a life-threatening disease or illness and who wish to receive palliative care at home.
Under the program, eligible patients receive:
- coverage of medications used in palliative care through the PharmaCare BC Palliative Care Drug Plan, and
- medical supplies and equipment through the local health authority.

BC Palliative Care Benefits Program Application Form (PDF 381K) (HLTH 349) – updated June 2011. Replaces the previous version (2006/07/17). For ease of reference, the new form incorporates the Palliative Performance Scale (PPS) with instructions for its use; physicians no longer need to refer to external documentation for this information. The form has also been redesigned to make it easier to complete and use. Note: The form contains no new eligibility criteria for requesting palliative care services.
