Special Notes
- Botulinum toxin for cosmetic purposes and indications not approved
for sale by Health Canada are not eligible for PharmaCare coverage.
- For renewals, physicians must submit
- documentation of a patient's functional and symptomatic improvement,

- dosage and injection schedule
